Meibomian Gland Dysfunction, also known as MGD or Posterior Blepharitis, is where the meibomian glands in your eyelids are not working like they should.  The meibum (oil) that these glands produce is used to moisturize and lubricate the eye.  So without meibum your eyes will feel dry, irritated, gritty, and dry eye syndrome will develop.  Dry eye is just a manifestation in a continuum of diseases starting with inflammation in the body, and with the eyelids, blepharitis and meibomian gland dysfunction.  

There are many ways to treat meibomian gland dysfunction naturally and at home, which I discuss in great detail in another article [Click Here to Read]. 

In today's article I will be discussing an in-office procedure called BlephEx, which is used to clean the eyelids.    

The Researchers

In a recent study, James Rynerson, MD, and Henry Perry, MD proposed that dry eye and blepharitis are the same disease process.  Dr. Rynerson states "We get biofilms, they lead to inflammation, and then the inflammation affects the different structures in the lid."   This, they proposed, is what leads to dry eye, decreased contact lens wear time, and decrease tear breakup time (TBUT).  

Increasing Contact Lens Wear Time 

Patients believe that as they age, they have to discontinue contact lenses due to irritation and dryness that occurs.  Practitioners over and over again will switch solutions and switch contact lens brands to try and keep these patients in contact lenses. 

Dr. Rynerson reveals that these practitioners should be focusing on cleansing the eyelid margins and getting rid of the biofilm formation.  Once biofilm is removed, the source of inflammation goes away, and an immediate relief can be felt from the patients.  

In their practice they use BlephEx, which is a hand--held instrument with a spinning sponge soaked in cleaning solution that removes biofilm from the patient's lid margins and reduces inflammation.  

"Once you remove the biofilm, the source of inflammation goes away, and patients begin to feel so much better immediately," Rynerson said.  

TBUT (Tear Breakup Time)

Tear breakup time is when a practitioner looks at the tears to assess the quality.  When a patient blinks, the tears should hold steady for at least ten seconds.  With a less than ten second TBUT, treatment with eyelid cleansing should be initiated.  

Studies with BlephEx showed a 60% increase in TBUT.   An increase in TBUT reveals an increase in meibomian gland function.  This study evaluated 20 patients at initial examination and at four weeks post BlephEx treatment.   Another study showed a 66% increase in TBUT and evaluated 90 patients. 

My opinion on this study and the data

These results are great, and I have used BlephEx first hand in my practice with the success stated in the articles. 

My recommendation for treatment and the use of BlephEx is to find a doctor in your area and have BlephEx done every four-to-six months depending on your severity of dry eyes, blepharitis, and/or meibomian gland dysfunction. 

In between visits, using other natural therapies as a compliment to BlephEx will promote even better results. I recommend the following: 

1. Begin with a warm compress

2. Safely express the meibomian glands

3. Clean your eyelids twice per day with a hypochlorous acid based solution

 4. Eat healthy omega-6 and omega-3 fatty acids

Taking Proper Care of Your Meibomian Glands

How often should you manually express your Meibomian glands? Is warm compress and massage different from gland expression?

We'll quickly go over what we recommend. At night, use our Heyedrate Dry Eye Mask or another warm compress heated up in the microwave for a 15-20 seconds. Lie it over your closed eyelids for 10-15 minutes. What that really does is it loosens up those oils that are in the meibomian glands. Sometimes those oils can be impacted just because they’ve been in there for so long and they’re poor quality. We want to get those out, so by heating it up, that’s the first step.

Lid massage and meibomian gland expression are slightly different. After warm compresses, you can do a little lid massage by gently pushing on your eyelids a little bit to break up any of the impacted oils. Gland expression is similar to lid massage it’s just we tend to sandwich the eyelids. This is usually done in an eye doctor's office. We put something behind the eyelids and we put something in front of them and then we milk the glands. You can do this yourself with a q-tip if you want to... just take a q-tip over your closed eyelids and roll it down your Meibomian glands. Do this from the top to the bottom lids.

We tell patients it's hard to overdose on warm compresses and gland expression. You can do this a few times per week unless you have very severe MGD, then you may need to visit your eye doctor to have this done often.

Hey, Dr. Travis Zigler here with Eye Love. I wanted to go over how blepharitis and dry eye are related. A lot of people think they’re separate conditions, which technically they are, they are two different things. Blephra being your eyelids, dry eye being on the eye itself, but very related in symptoms, signs. Their together in almost every way. Blepharitis is inflammation of the eyelid. You can have two different types blepharitis. You can have anterior and posterior blepharitis. Anterior blepharitis tends to be more bacterial related. We have normal bacteria that lives on our eyelids and they can become over populated. When that normal bacteria becomes over populated it’s going to cause some signs and symptoms. Your eyes are going to be red, your eyelid is going to be red and itchy. It’s going to be inflamed and it’s going to be irritated. That’s when you see the kind of dandruff looking things on your eye lashes. That’s an anterior blepharitis, meaning it’s more of a bacterial infection. It’s a normal bacterial infection, just your normal bacteria that lives on your skin is over populated. That’s where the spray comes in. This spray is a natural antibiotic. It will never develop resistance and it keeps that normal bacteria under control. Keeps the eyelid clean without disrupting the surface too much. Now, on the other side posterior blepharitis is what we commonly call meibomian gland dysfunction. Meibomian gland dysfunction is where the eyelids, on the inside, there’s oil glands, there’s about 31 on top and 31 on bottom. The oil glands aren’t getting enough oil out to make your eye comfortable and the responsibility of that oil is to make your eye comfortable. Every time you blink the oil is released. Now anterior, posterior blepharitis are both related though, because if you have a bad anterior surface to the eyelid it’s gonna disrupt the posterior surface too. What can happen is if these aren’t healthy, the oil will get broken down before it gets to the surface and it might even create kind of more of a detergent, or a bad effect on the eyelid. The oil will break down into a more severe form, and not a very good form, that will cause more irritation which then leads to dryness. So, if you get your eyelid healthy it’s going to make your dry eye better. Which is why we came out with this. This is going to help keep that bacteria under control. It’s going to help the meibomian glands open and working properly. It’s just going to help your eyes stay comfortable overall.

So, how do you use this? I’ve done other videos showing on it, but I’ll show you briefly. I do it differently than my wife. I take it after I wash my face and wash my eyelids like I normally do at night. I just take it, spray it directly on, and then I rub it in. If it gets into your eye it's not a big deal. Now, what I find most of my female patients like to do is they like to take a cotton round or a cotton ball, spray 3 to 4 sprays, and then go side to side, and up and down, nice and gentle. You don’t have to scrub too hard. This again, can replace the accusoft lid scrubs that your using currently and this is much more gentle because it doesn’t have any surfat or surdetergents in it, to disrupt that normal flora or the normal bacteria that live on your eyelid. This will actually keep all that working really well. So, blepharitis and dry eye are different conditions but one in the same. If you have one, you usually have the other, and if you get blepharitis under control you can usually get your dry eye under control. That’s why we are pushing so hard for the spray and also for the omega-3 supplement. Posterior blepharitis helps with the anterior and posterior blepharitis. This one helps with everything though.
